Please initial and sign where noted and make copy of these documents for you records.

At the time of my pets’ untimely death, I understand required to take
(Initial) my pet to my veterinarian immediately to determine cause of death, as well as the date and time of death.
     
 

I understand that I am required to call or email PETLIFE within five (initial) days of the date of my loss.

  After we are contacted, you and your veterinarian will promptly receive a payout form at which time you and your veterinarian will fill out completely. (It is vital that your veterinarian fill out his/her portion of documents as it will greatly and subsequently determine whether or not you will receive any compensation).

I understand that I must send it back with any additional information (Initial) required by PETLIFE within 3 days.     

We then will review all documents and you will hear from us by mail, e-mail, or by phone within 48 hours. If at that time we need any further documents, you will be notified and will be instructed what to do.  If no other documents are needed, you will be mailed a compensation check with our sincere sympathy in a timely manner (allow up to ten days).  This we believe this to be the most efficient way of a payout at this time, if however we see a more efficient way, we reserve the right to change this procedure accordingly.  PetLife offers an apology for any inconvenience of this procedure but at this particular time it is the best way.

As well as adhering to the payout procedure, I will also to my pet  (Initial) to regular check-ups, and maintain a good status with a licensing vet.

I have read the above document and do understand the responsibility I must have regarding the untimely death of my pet. And I will adhere to all that is required of me.
